IC3 Beacon Community Overview
In April, 2010, Utah was named one of 15 Beacon Communities by the Office of the National Coordinator for Health Information Technology. Two Communities were subsequently identified in September for a total of only 17 nationwide. This effort, “Improving Care through Connectivity and Collaboration” (IC3) will engage key partners
(healthcare providers, public health and policy) within the community to use Health Information Technology to:
- Demonstrate that health IT-enabled quality, cost/efficiency, and population health improvements are possible in diverse communities, within the program period (3 years);
- Support lasting “innovation networks” in our community through which a wide range of stakeholders can continue to collaborate, design, and implement new technology-enabled ideas that improve health and health care beyond the program period; and
- Provide lessons, implementation insights, and best practices for other communities eager to improve health, health care, and cost-efficiency in their communities.
Benefits of Participating
Benefits of being an IC3 participating provider include:
- cHIE connection: Financial assistance available for a limited time to support connection to the state clinical health information exchange (cHIE)
- Meaningful Use Support: Priority assistance from the Regional Extension Center in achieving meaningful use requirements for federal incentives
- Care Process Redesign: Analysis of and support in improving current processes for managing the care of patients and other chronic illnesses
- Diabetes Care Coordination: Offers the opportunity for improved communication between providers
- Community-wide Collaboration: Learning and networking opportunities that support clinical process improvement, improve health information flow, and encourage community-wide adoption of best practices
- Tools and Resources: Access to electronic decision support tools, as well as patient education and self-management resources
- Quality Data Feedback and Benchmarks: Access to clinical quality improvement data and benchmarks on diabetes measures
- Beyond Diabetes: Improved connectivity to public health systems
Who Can Participate?
- Primary Care Providers with electronic health record (EHR) systems
- Diabetes Specialty Providers
- All providers must have a willingness to connect to the cHIE, achieve Meaningful Use, participate in diabetes quality improvement and community learning activities, and participate in data sharing.
