Chronic Kidney Disease affects one in nine Americans
| HealthInsight and a statewide network of health care partners are collaborating to take on chronic kidney disease (CKD). Together, we will educate at risk patients and promote timely screening and preventive measures to slow the progression of this rising nationwide healthcare issue. - Statistics now show that 26 million Americans have CKD and another 20 million are at increased risk.
- The early stages of CKD often have no outward symptoms, and damage to the kidneys is irreversible.
- Early screening and education for patients will save lives.
- African Americans, Hispanics, Pacific Islanders, Native Americans and Seniors are at increased risk.
- In the state of Nevada, the incidence of CKD is one in every seven adults.
- In the state of Utah, roughly one in eight adults have CKD.

Overview
The two main causes of chronic kidney disease are diabetes and high blood pressure, which are responsible for up to two-thirds of the cases. Having a family history of kidney disease, being older or belonging to a population group that has a high rate of diabetes or high blood pressure places you at increased risk for kidney disease.
HealthInsight, Nevada and Utah, are two of the ten states awarded a Centers for Medicare & Medicaid Services contract to undertake an initiative to address this growing concern with a goal to make statewide gains on the following measures (click here for details):
- Timely testing to reduce the rate of kidney failure due to diabetes;
- Slowing the progression of this disease in individuals with diabetes through the use of ACE inhibitor and/or an angiotensin receptor blocking (ARB) agent; and
- Arteriovenous fistula (AV fistula) placement and maturation (as a first choice for arteriovenous access where medically appropriate) for individuals who elect, as part of timely renal replacement counseling, hemodialysis as their treatment option for kidney failure.

How will we accomplish our goals?
- We have adopted the Chronic Care Model as a foundation for accomplishing our goals.
- We have assembled a group of statewide organizations and individuals with similar goals.
- We will base care around evidence-based guidelines for the identification and treatment of chronic kidney disease.
- We will recruit a select number of Primary Care Physician Offices to provide feedback on interventions to reach this at-risk population.

- We will assemble educational materials that will carry strong key messages for providers and consumers on the importance of recognizing and collaborating on the identification and treatment of chronic kidney disease.
How will we monitor our progress?
- We will share with our partners, on an on-going basis, the Centers for Medicare & Medicaid Services statewide rates for our clinical measures.
- We will dialogue with our partners on an on-going basis to learn about individual, office and organization successes in reaching and assisting our at-risk individuals.
- We will share best practices via newsletters, articles and other media.
