Improvement is based on intentional efforts directed at specific areas within the practice’s care processes. Best practices, clinical guidelines, and standards of care are a few suggestions for beginning an investigation into current levels of achievement in a specific area of patient care.
Whether or not the practice is currently using an EHR system is unimportant to the process of improving clinical care. In every case, the first step is to identify an area of clinical care to improve. Generally, baseline data is required to assist in identifying an area for improvement and allows the practice to measure the improvements gained as a result of intentional interventions targeting clinical care.

| Many Utah clinics are participating in the Medicare Care Management Program (MCMP), a pilot program intended to improve care for Medicare beneficiaries with chronic diseases. Clinics will be collecting specific measure of care for patients with diabetes, heart conditions, and some preventive services. Clinics without an EHR can use flow/data collection sheets to document the measures. |
