HOME ► Home Health Home Health Submit a Success Story | Read Local Success Stories Home Health Quality Improvement National Campaign 2007Home health patients desire and prefer to stay home whenever possible. Hospitalizations can unnecessarily create financial and emotional burdens for patients and their families, and can negatively impact the health care delivery system. Currently, more than one in four home health patient episodes will result in a hospitalization. Home health quality improvement interventions can be effective in reducing avoidable hospitalizations. The Home Health Quality Improvement (HHQI) National Campaign 2007 seeks to unite the home care community under the shared vision of reducing avoidable hospitalizations to improve patient quality of care. The Campaign is utilizing a 12-month multi-disciplinary approach to quality improvement that includes key home health, hospital, and physician stakeholders. During the campaign, home health agency recruitment and communication will occur locally through state and national home health associations and Quality Improvement Organizations (QIOs). These groups will partner to serve as Local Area Networks for Excellence (LANEs). The campaign will provide home care agencies with free monthly best practice intervention packages that include educational tools and resources, guidelines, success stories and best practice education that may be used to assist agencies in reducing avoidable hospitalizations. National Goal for Home Health AgenciesThe goal for the campaign is to increase the number of patients receiving home health care who will remain in their home setting by reducing the number of patients requiring avoidable hospitalizations. The success of the campaign will be measured by the average success of the participating home health agencies. OASIS data collected for each Medicare and Medicaid patient served by the participating agencies will determine a participating agency’s average acute care hospitalization (ACH) rate.
