2008 Nursing Home Award Criteria

2008 HealthInsight Quality Award Criteria – Nursing Homes

Nursing Home recipients of the 2008 HealthInsight Quality Award will be selected based upon the following criteria:

Part One:
Using the publicly reported data that appears on www.medicare.gov, ALL of the long stay quality measures with the exception of the two pressure ulcer measures will be selected for analysis.  These measures are:

  • Percent of residents given the influenza vaccination
  • Percent of residents who were assessed and given the pneumococcal vaccination
  • Percent of residents whose need for help with daily activities has increased
  • Percent of residents who have moderate to severe pain
  • Percent of residents physically restrained
  • Percent of residents who are more depressed or anxious
  • Percent of residents who lose control of their bowels and bladder AND who have a catheter
  • Percent of residents who spend most of their time in bed
  • Percent of residents whose ability to move around in their room has gotten worse
  • Percent of residents with a urinary tract infection
  • Percent of residents who lose too much weight

Facilities that have obtained a ranking at or above 75% will meet part one of the award criteria.

Part Two:
In order to meet the second criteria for the nursing home award, facilities must have at least TWO of the following quality markers firmly in place (as assessed by HealthInsight), These patient-centered quality markers are:

  • Permanent assignment (also known as consistent resident assignment) is when a nursing home has their nursing staff permanently assigned to a unit, neighborhood, or household at least 75% of the time.
  • Resident choice - resident-centered care delivery involves providing opportunities for nursing home residents to make their own choices such as being able to decide when they would like to wake in the morning.  Facilities must allow all residents to go to bed and wake up when they like in order to be included in this marker.
  • Dining - resident-centered care delivery involves providing nursing home residents with a dining experience that resembles the homelike mealtime experience.  Facilities must provide buffet style, family style, or restaurant style dining for any one of the main daily meals to be included in this marker.
  • Multi-disciplinary rounds are when a set group of staff from a variety of disciplines walks through the nursing facility to discover any issues that need quick resolution and gain situational awareness.  This marker specifically requires facilities to conduct rounds that include staff members who are outside of his/her discipline (i.e., housekeeping employee conducting rounds for, or with, dietary or nursing staff).

Part Three:
In order to receive the 2008 HealthInsight Quality Award, facilities must also demonstrate their efforts to foster relationships with their community hospitals that display strong communication and systematic processes to eliminate harm, waste, and error.
